Hours: Primarily Days and limited evening hours

Position will primarily work days and limited evenings in order to accommodate the need of working participants. 

 

Must have a minimum of a Bachelor?s degree in behavioral health or, a health and/or wellness discipline, with at least 2 years of full-time experience in a healthcare or wellness setting; or a Master?s degree in helping profession. Prior experience providing health coaching or health education preferred; or experience providing primary counseling services to adults. Must be able to use standard computer technology. Comfortable with face-to-face and telephone-based services, self directed and able to provide a high level of customer service to corporate customers; willing to adhere to stated program contact and documentation protocols.

 

With stated Protocols, schedule and complete a variety of outreach, assessment, follow-up and case coordination activities to participants of a disease management program. Maintain a case record of all contact activities with participants in an accurate and timely manner. Assist in the quality assurance and compilation of all such data. Accurately collect data and re-assessment information for all participants in accordance with the designed schedule.
