Hours: Primarily Days and limited evening hours

 Position will primarily work days and limited evenings in order to accommodate the need of working participants. 

 

Registered Nurse with two years of experience; Or a Bachelor?s degree in a behavioral health or a health and wellness discipline, with extensive experience in a medical practice setting and a demonstrated working knowledge of the treatment of chronic medical conditions such as diabetes, cancer and/or cardiac issues. Prior experience providing health education preferred. Must be able to use standard computer technology.

 

Prior experience with the following is also helpful: assisting patient with treatment compliance; working in a behavioral health capacity; understanding of behavioral change strategies such as Motivational Interviewing. Comfortable with face-to-face or telephone-based services, able to develop and provide presentations on wellness; self-directed and able to provide a high level of customer service to corporate customers; Able to adhere to stated program contact and documentation protocols.

 

With stated Protocols, schedule and complete a variety of outreach, assessment, follow-up and case coordination activities to participants of a disease management program. Maintain an understanding of program structure and available resources outside the program for primary care of participant concerns. Facilitate referrals to treatment providers or other resources in accordance with program protocols.
