Hours: Mon-Fri, 8:00 a.m. to 5:00 p.m. with rotating on-call shifts

Master's degree in Social Work or closely related human service field plus 1 year experience required.  State of Illinois licensure as LPC or LSW required.  Proven ability to consistently meet agency productivity standards for Case Management Program is required. Must have have keyboarding and computer skills as required by position. Must have valid IL driver's license, clean driving record, reliable transportation, and proof of automobile insurance.

Direct the day-to-day clinical operations of the ACT Team, supervise movement of clients within the ACT program, ensure ACT documentation and compliance with all funding sources and agency policy and procedures. Coordinate and implement staff schedules (i.e. evening and weekend schedules), Ensure that ACT staff productivity standards are met.

Participation in the ACT after-hours on-call schedule; e.g. evening and weekend rotatio, including after-hours hospital screenings is required.
