Basic Function:



  • Assumes the responsibility for providing skilled nursing, assessment, and supportive care at the patient?s residence during evening, nights, and on weekends. 

  • Works on own with little access to office staff and limited access to physicians and supervisors.

  • Triages physician and patient concerns. This includes during the night time hours.  Confident with the ability to exercise excellent nursing judgment during on-call hours, utilize past experience and current knowledge to communicate with physician.

  • Admits patients to agency and provides follow-up visits at needed.  May be required to visit places hard to find at night and experience difficult situations. 

  • Able to convey a sense of compassion at all times regardless of the situation.

  • Knowledgeable of Joint Commission Standards and CMS criteria

  • Demonstrates Memorial Health System?s Mission, Vision and Value Statement as well as Behavioral Standards.


 


Requirements:


 



  1. Registered Nurse, licensed to practice in IL.

  2. 2-3 years experience in acute care setting and 2+ years in Hospice required.

  3. Demonstrated expertise and ability to work independently with excellent communication and problem solving skills.

  4. Demonstrates strong assessment skills and ability to copy with patient/family emotional stress and tolerance of individual lifestyles.

  5. Demonstrated ability to respond in urgent situations with minimal supervision and support. 

  6. Demonstrated ability to function independently with minimal supervision and exercise excellent critical nursing judgment.

  7. Knowledge of hospice regulations; carries a cell phone and is responsible for all hours on call that may require visits anywhere within Home Hospice Services territory. 

  8. Current CPR certification

  9. Must possess valid IL drivers license and must be deemed as an acceptable driver in accordance with MHS Fleet Safety Policy (5 year MVR will be required).  Must have personal vehicle but may be required to accept company car if mileage warrants.

  10. Requires full range of body motion including handling and lifting patients; Occasionally lifts and carries items up to 50 pounds.
