8 a.m. - 5 p.m. and rotational weekends on-call
Basic Function: Nursing professional ensuring coordination and continuity of care for pre- and post transplant patients, as well as living organ donors. The skilled transplant coordinator works with physicians, dialysis units and hospital personnel in a team effort to identify, prepare and educate the potential transplant recipient and potential living organ donors. Collaborates care with transplant attending surgeon(s), transplant physician(s), other attending physicians, surgical and physician residents, floor nurses, transplant interdisciplinary team members and outpatient clinic personnel. Provides patient nursing assessment based on social, developmental and physiological data. Provides care for transplant patients and living organ donor patients within protocols developed by transplant team under direction from Medical Director. Provides patient care and data tracking in accordance with appropriate regulatory guidelines (e.g., CMS, UNOS) This position requires independence, efficiency and attention to detail. Must be able to rotate a 24/7 on-call responsibility.
Requirements for this position include:
1. Registered Nurse with current State of Illinois license.
2. Degree from an approved Baccalaureate Nursing Education program preferred.
3. Two years experience in the care of nephrology and/or transplant patients required.
4. Nursing experience in an acute care setting preferred.
5. Comfort with and proficiency with computer and internet required.
