The Social Worker provides psychosocial assessment and supportive counseling to patients and families to facilitate clinical improvement and discharge. Services provided include: psychosocial assessment; supportive counseling; grief counseling; advance directive counseling; chemical dependency screening and referral; processing of guardianship issues; identification and crisis intervention counseling for victims of abuse/neglect; awareness of and referral to community resources for specialized psychosocial needs. All activities are focused on assisting the patient, family, and team to address psychosocial issues to ensure a smooth transition from inpatient to community-based settings.
Requirements for this position include:
- Master of Social Work (MSW) degree from school accredited by Council on Social Work Education, or completion of MSW within five years of hire if holder of Bachelors of Social Work (BSW) degree.
- Illinois Licensed Social Worker; Licensed Clinical Social Worker preferred.
- One (1) year clinical experience in multidisciplinary medical setting preferred.
- Strong oral and written communication skills.
