ALMH - RN Care Coordinator In Illinois Job Details

Hours: 3:00 p.m. - 11:30 p.m. and rotational weekends on-call

Basic Function: 

To provide clinically-based screening and care coordination to assist in the identification of appropriate utilization of resources in the hospital setting.  Case Management ensures a continuum of care for each patient from infant to elderly, which begins in the pre-admission or admission stage and is developed through hospitalization, into the appropriate level of care. 

Specifications:  Knowledge, Training, Work Experiences:

1. Graduate of an accredited school of nursing meeting licensure requirements in the State of Illinois.


2. Current CPR certification per policy.  ACLS preferred.


3. 3-5 years of recent clinical experience.


4. Case Management/Utilization Management experience preferred.


5. Excellent time management and organizational skills.


6. Effective written and verbal communication skills.  


7. Nursing Supervisor or Charge Nurse experience strongly preferred


8. CAH experience preferred.


9. Knowledge of regulatory agency guidelines (IDPH, CDC, Insurance, etc) required.
