Hours: Monday thru Friday, 8:30 a.m. to 5:00 p.m.

Bachelor's degree required, preferably in social services field, or meets the ?Mental Health Professional? designation as determined by the Department of Human Services and have prior experience working with persons with mental illness.Must have keyboarding and computer skills as required by position. Must have valid IL Driver?s License, reliable transportation, and proof of auto insurance. Must have or obtain CPR, First Aid, and CPI certifications.

Provide practical assistance to clients to improve their ability to cope within the community within the least restrictive setting possible (for example, assistance may be needed with transportation, budgeting, locating housing, and/or identifying vocational training opportunities).

Support residents in taking medications as prescribed and in following other medical directions.  Monitor compliance and document, as required.

Provide case management assessment, monitoring, linkage, support, and advocacy.  Refer to, and work close with, appropriate agency and community providers to assure linkage of client to needed services and resources.
