Hours: 8:00 a.m. - 4:30 p.m.; Every 6th Weekend Plus Call

Basic Function:

Provides palliative nursing care to Hospice patients through the use of the nursing process and in accordance with standard nursing practice and established policies and procedures.  Assumes full responsibilities for direct patient care, patient evaluations and admissions.

Specifications:

Must be licensed to practice professional nursing in Illinois, a BSN is preferred.  A minimum of 2 years clinical experience mandatory; one year in home health nursing or previous hospice experience preferred.  Must provide own transportation to and from patient?s home. Must possess valid Illinois drivers license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must have availablity of personal vehicle with proof of insurance. Current CPR certification per policy. Must be able to position and/or transfer an average adult patient and possess gross coordination sufficient to perform other patient care duties as required.  Environmental working conditions include exposure to blood, bodily fluids and tissue, contagious diseases or other potentially hazardous materials.
