Bachelor's degree required, preferably in social services field, or must meet the "Mental Health Professional" designation as determined by the Department of Human Services by having 5 years verifiable experience working with mentally ill individuals. Computer/typing skills required.  Assertiveness and ability to work independently required.  Related experience preferred.  Must have valid IL Driver's License, clean driving record, reliable transportation, and proof of automobile insurance.

Must be able to provide at least 25 hours of direct service to clients each week.

Provides community-based case management services for mentally ill adults in the Community Support Services Unit.  Ensures continuity of care for high-risk persons suffering from mental illness through direct clinical intervention, monitoring, and advocacy functions.
