Registered Nurse - Memorial Home Services In Illinois Job Details

Provides palliative nursing care to  Hospice patients, after hours, through the use of the nursing process and in accordance with standard nursing practice and established policies and procedures.  Assumes full responsibilities for direct patient care and follow-up with Hospice Primary Nurse or RN Office Field Coordinator when shift has ended.    Interfaces with Hospice team members to provide optimal, emotional, physical and spiritual care for the terminal patient and family members, through voicemail or email,  Conducts evaluations and admits patients to the hospice program according to hospice guidelines and procedures and reimbursement guidelines.  Makes contacts with Physicians as needed.

Requirements:

1. Must be licensed to practice professional nursing in Illinois.  BSN is preferred. 


2. A minimum of 1 year of home health or hospice experience required.


3. A minimum of 2 years clinical experience is required. 


4. Excellent communication skills and customer service skills. 


5. Current CPR certification is required.


6.  Must possess valid Illinois drivers license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must have availability of personal vehicle with proof of insurance.


7. Familiar with Medicare, Medicaid, and other insurance requirements.


8. Proactive, positive, problem-solving skills.


9. Demonstrated ability to be a team player.


10. Basic computer skills required.
