Hours: 8:00 a.m. to 4:00 p.m. M-F with night & weekend PRT shifts

This Position is located in Lincoln, IL. 

Bachelor's degree required, preferably in social services field or must meet the "Mental Health Professional" designation as determined by the Department of Human Services by having 5 years verifiable experience working with mentally ill individuals. Computer/typing skills required. Assertiveness and ability to work independently also required. Related experience preferred. Must have valid IL Driver's License, clean driving record, reliable transportation, proof of automobile insurance, and be able to pass DCFS background check.

 Must be able to provide at least 25 hours of direct service to clients each week.

Provides community-based Case Management services for mentally ill adults and behaviorally disturbed children and family or caregivers in the DHS SASS Programs. Ensure continuity of care for high-risk persons suffering from mental illness through clinical intervention, monitoring, and advocacy functions.

Must be able to participate on the Psychiatric Response Team (PRT), the agency's after-hour crisis intervention services as requested by supervisor.
