Basic Function:


Develops, plans and implements the Home Health and Hospice programs as they relate to patient care services and departments.  Responsible for daily operations of Home Health (skilled nursing, physical and occupational therapy) and Hospice (nursing, social services, bereavement) departments.  Participates in long range strategic planning for department and agency.  Manage the areas of safety, compliance, infection control, intake, case management, JCAHO, Public Health, customer satisfaction and quality management of Home Services division. Fosters a cooperative relationship within Memorial Health System affiliates.


Requirements:



  1. BSN required and current license to practice professional nursing in Illinois.

  2. A minimum of ten years Home Health and Home Hospice supervisory experience is required.

  3. Knowledge of regulations related to home services and experience in managing       compliance activities for Home Health and Home Hospice clinical operations.

  4. Excellent interpersonal, oral and written communication skills.

  5. Demonstrated ability with problem-solving and decision-making to develop innovative approaches to a changing home health care environment.

  6. Knowledge of Quality Improvement philosophy and process.

  7. Experience with case management functions, coding and billing functions. 

  8. Demonstrate strong process management skills and ability to work collaboratively with multiple constituencies.

  9. Familiarity with quality improvement processes, data collection and data analysis tools.

  10. Experience with JCAHO and Public Health surveys and standards preferred.

  11. Possess a working knowledge of computer skills (Microsoft Office and control chart software) preferred.

  12. Universal Precaution Category I.
