Basic Function:
Provides nursing care to Home Health patients through the use of nursing process in accordance with standard nursing practice and established nursing policies and procedures. On Call for home health services. Triages patients/physicians concerns. Admits patients to agency and/or provides follow-up patient visits as needed, in accordance with agency policies and procedures and reimbursement guidelines. Provides communication to Clinical Supervisors, schedulers, and Home Health nursing staff as appropriate. Makes contact with physicians as needed for patient care.
Requirements:
- Must be licensed to practice professional nursing in Illinois. BSN is preferred.
- A minimum of 2 years home health experience is required.
- A minimum of 2 years clinical experience is required.
- Excellent communication skills and customer service skills.
- Current CPR certification is required.
- Must possess valid Illinois drivers license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must have availability of personal vehicle with proof of insurance.
- Familiar with Medicare, Medicaid, and other insurance requirements.
- Proactive, positive, problem-solving skills.
- Demonstrated ability to be a team player.
- Basic computer skills required.
