Basic Function: Provides palliative nursing care to assigned Hospice patients through the use of the nursing process and in accordance with standard nursing practice and established policies and procedures. Assumes full responsibilities for direct patient care and follow-up with Hospice Primary Nurse or appropriate individual for caseload assigned that day. Supervises care rendered by Home Health Aids as needed. Interfaces with Hospice team members to provide optimal, emotional, physical and spiritual care for the terminal patient and family members. Conducts evaluations and admits patients to the hospice program according to hospice guidelines and procedures and reimbursement guidelines. Candidate will be providing patient care in the Macon County Region.

Requirements:

• Licensed to practice professional nursing in Illinois, BSN preferred


• Minimum of 2 years clinical experience mandatory, one year in home health nursing or previous hospice experience preferredMust provide own transportation to and from patient's home, medical facilities, etc.


• Provide proof of current valid driver's license and current valid automobile insurance
