Responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines.  Graduate of an approved school of Physical Therapy.  Illinois eligible or licensed Physical Therapist.  Basic knowledge in physical therapy practices in accordance with P.T. Practice Act.  CPR certification preferred.  Universal precautions Category I.  Current Drivers License and automobile insurance required.
