Hours: M-F 0830-1700

Bachelor's degree required, preferably in social services field, or must meet the "Mental Health Professional" designation as determined by the Department of Human Services. Related experience preferred.  Must have excellent computer and communication skills.  Must be able to participate in agency's after-hours crisis intervention services as requested by supervisor.  Must have reliable transportation, clean driving record, proof of automobile insurance, and be able to pass DCFS fingerprinting/background check.

 

Provides community-based daily case management services to emotionally and behaviorally disturbed youth and their birth families and/or substitute care providers in the DHS SASS Programs.  The SASS Case Manager will insure continuity of care for high-risk youth suffering from emotional and behavioral disturbances through direct clinical intervention, monitoring, and advocacy functions.  Must be able to meet productivity requirement by providing at least 25 hours of direct service each week.
