We are seeking a dynamic APN for our Heart Failure division. Candidate can either be a Clinical Nurse Specialist OR a Nurse Practitioner.
Nurse Practitioner: Facilitates the delivery and evaluation of evidence-based comprehensive care to the inpatient and outpatient heart failure population. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role. Under the direction of the heart failure physician or designee, provides coordination of the heart failure clinic.
Requirements of the position include:
1) Certification as a Nurse Practitioner.
2) Must possess and maintain licensing as required by Illinois State law for APN.
3) Minimum of three years post-school experience as an APN.
4) Three-Five years experience in Cardiology or ICU nursing.
5) Valid CPR & ACLS certification required.
6) Maintains DEA license.
Clinical Nurse Specialist: The Clinical Nurse Specialist/Care Manager operates within the MHS's corporate and hospital philosophy, goals and objectives. The Clinical Nurse Specialist/Collaborative Care Manager promotes high quality patient focused cost-effective patient management across the continuum of care for designated high-risk complex patient populations. Responsible for providing and/or coordinating comprehensive care appropriate to the age/population of patients served. Accountable for questioning, investigating, developing, implementing and evaluating cost-effective programs/interventions which promote quality patient outcomes. Also accountable for collaborating with the health care team to define and communicate the expected patterns of care for complex patient types. Provides leadership for education and research programs to support the advancement of professional nursing practice and improvement of patient outcomes. Acts as a resource, leader and role model for professional practice throughout the healthcare system. Functions as a liaison with the community, other health care professionals, and other disciplines regarding professional nursing practice and areas of clinical expertise.
Requirements of the position include:
1. Current licensure as an RN in the State of Illinois.
2. A Master's degree in nursing with a focus on specialty clinical practice. CNS and CCRN or pursuance of CNS and CCRN required.
3. Five years experience in clinical nursing with in-depth, current knowledge and competency in specialty. (Three years of critical care experience preferred.)
4. Certification by AHA as a CPR Instructor-Trainer and ACLS Instructor.
5. Excellent interpersonal, communication and customer service skills.
6. Demonstrated leadership in organizational change and quality management.
7. Evidence of continuing professional development.
